Introduction

Many of our patients at Fab Eye get a red eye and wonder, am I getting pink eye? We are here to help and explain what could be going on. People often refer to an acute red eye as “pink eye”. What is pink eye? For the most part pink eye is a common cause of urgent ophthalmic visits and encompasses a range of conditions affecting the conjunctiva. While often self-limiting, certain etiologies require prompt diagnosis and intervention to prevent complications and also reduce transmission. As a consequence patients in the Philadelphia suburban area, including Malvern, Paoli, Exton, West Chester, and Phoenixville, frequently present to urgent eye care for this condition in our office.


Etiologic Categories for Pink Eye

  1. Viral
    • Most commonly caused by adenovirus serotypes.
    • Transmission is highly contagious via direct contact or fomites.
    • Frequently associated with upper respiratory tract infection.
    • Clinical features: watery discharge, follicular conjunctival reaction, unilateral onset with contralateral involvement within 24–48 hours.
  2. Bacterial
    • Common pathogens: Staphylococcus aureusStreptococcus pneumoniaeHaemophilus influenzae.
    • Typically presents with mucopurulent discharge, eyelid crusting upon awakening, and conjunctival injection.
    • In severe cases, hyperacute onset may be caused by Neisseria gonorrhoeae, requiring immediate systemic therapy.
  3. Allergic
    • IgE-mediated hypersensitivity reaction to environmental allergens like pollen, animal dander, and dust mites.
    • Often bilateral with marked pruritus, tearing, and papillary conjunctival response.
    • May be seasonal (hay fever conjunctivitis) or perennial.
  4. Irritant
    • Chemical, smoke, or particulate exposure causes direct conjunctival irritation.
    • Symptoms resolve with removal of the offending agent and supportive care.

What is the cause of the infamous “I’ve got Floaters”? It is usually a diagnosis of Posterior Vitreous Detachment, that is often a benign finding. However, it can be a warning sign of more serious complications. One symptom patients experience with a “PVD” are floaters which are shadowy shapes that appear like they are floating in your line of vision. These shapes can look like spots, cobwebs, or bugs. Other symptoms are blurry or hazy vision, flashes of light, or what appears to be a shadow or curtain anywhere in the field of vision. Patients should contact the office if any of these symptoms develop to rule out other retina issues that can come along with a posterior vitreous detachment.

We diagnose a PVD through a fully dilated eye exam. If no retinal break is found, the patient will be scheduled for a follow up examination with scleral depression in 2 to 4 weeks, 2 to 3 months, and 6 months after symptoms first develop. If no retinal break is found, but mild vitreous hemorrhage or peripheral punctate retinal hemorrhages are present, follow up examinations are performed 1 week, 2 to 4 weeks, 3 months, and 6 months after the occurrence. If no retinal break is found, but vitreous hemorrhage or anterior pigmented vitreous cells are present, a repeat examination is performed the next day because there is a high likelihood of a retinal break.

How can Radiofrequency (RF) help dry eyes?

Our meibomian glands are located along the top and bottom eyelids and produce the oil layer of our tears. This oil helps keep the tears on our eyes. MGD causes glands to become inflamed, allowing less oil to be secreted into the tear film. Over time these glands can shorten and die off (atrophy) which causes our tears to evaporate more quickly and results in dry eye symptoms.

What can I expect?
OptiPlus RF is a comfortable, safe procedure that doesn’t have any downtime. Some patients report that it feels like a relaxing hot stone massage. After the procedure, you might have slight redness which is caused by your skin’s elevated temperatures during treatment and will subside soon afterward. You will be able to return to normal daily activities immediately!


How long does the treatment take, and how many do I need?
It’s typically completed in less than 20 mins! Because MGD varies in severity for each patient, multiple treatments are ideal to achieve optimal results. Your doctor will
recommend four treatments spaced about 2- 4 weeks apart to allow for the glands to begin flowing once again. The time between treatments will also allow time for your skin’s collagen to restore and rebuild elasticity before treating the area again.


Are there any side effects?
Patients may sometimes experience redness and slight swelling of the skin after a procedure. The redness typically fades within a few hours.
